Erika Smith

Erika Smith is the director of the organization's enterprise care coordination team made up of registered nurses, medical assistants and social workers. These clinicians support proactive population based outreach and care management. She also has a role with Inception Health with responsibility for digital therapeutics. Her background and expertise is in ambulatory pharmacy, population health and demonstrating value of new and innovative care teams. She has a passion for demonstrating and measuring value to be able to work towards scale of programs and models that support optimal patient and health system outcomes. She also is actively involved in national professional organizations such as ASHP and ACHE as well as has a passion for supporting her local community via volunteerism with United Way.
